Diagnosing nappy rash

How nappy rash is diagnosed

Diagnosis of nappy rash is usually based on the appearance of the rash and typically there is redness or a pink rash over the area closest to the nappy. Most cases of nappy rash are mild, but if the rash is severe the child may become distressed and uncomfortable.

Skin swabs are not usually required to make a diagnosis unless a secondary bacterial infection is suspected due to the severity of the rash.

Assessing a child with nappy rash

Some factors may predispose a child to nappy rash and these need to be assessed so that appropriate advice can be given:

  • Hygiene practices – how often the area is cleaned and the nappy changed should be established, as a lack of hygiene can predispose the skin to irritant dermatitis.
  • Friction – over vigorous cleaning can irritate the delicate skin
  • Exposure to irritants – soaps, detergents and alcohol-based wipes can all predispose the skin to nappy rash
  • Recent antibiotic use – antibiotics can predispose to candida colonisation
